Job Description
PURPOSE OF JOB: The outpatient Nurse Practitioner (NP) will provide comprehensive healthcare services to patients in the Palliative Care office and Patient Home settings. He/she will work as an integrated team member, fully responsible for his/her own clinic visits and for supporting and/or covering those of other outpatient providers as necessary. In addition, the NP will be a key member of the Palliative Care Service as a whole and will work with other Palliative Care staff. The NP will be highly professional, flexible and a team player.
- Provide palliative patient care in an outpatient office setting and in patient homes.
- Able to communicate effectively with providers, patients, nurses and other support staff verbally, in writing and/or via EMR.
- Maintains efficient and accurate use of time with scheduling, patient visits and EMR.
- Provide support to the Palliative Care team with regard to patient care and educational needs.
Job Qualifications QUALIFICATIONS :
- Masters of Nursing degree from an accredited institution required
- Current Pennsylvania licensure required:
- Certified Registered Nurse Practitioner (CRNP)
- Registered Nurse (RN)
- Prescriptive Authority
- Board certified NP
- Current BLS
- Experience: Previous experience as an NP in a Palliative Care Program preferred. Previous experience in Homecare or hospice required. Three year's experience as an NP required.
