SUMMARY (BASIC PURPOSE OF THE JOB)

Under the direction of the Director of Population Healthcare Management, manages the daily care management operations team for the acute care environments of both Regional and Hopewell Campuses. This includes ensuring cost effective and quality transition of care through the appropriate utilization of resources. Mentors, supervises and evaluates Care Management assigned staff. Collaborates with leadership on the implementation of performance improvement initiatives. Coordinates activity with Post Acute and Ambulatory Care Transitions team to create a patient centric process. Regularly monitors and assesses operations.

Minimum Requirements

Education: Bachelor's degree in Nursing (BSN), Master's degree preferred. Professional certification in one or more accepted specialties: Nurse Executive, Case Management, Healthcare Executive, or American Case Management Association (ACMA).

Experience: Five years case management nursing/utilization review experience or Five years acute care nursing experience and two years case management experience.

Other Credentials:Registered Nurse - NJ

Knowledge and Skills:Possess excellent communication, critical thinking, negotiation and conflict-management skills. Strong interpersonal and organizational skills. Possesses demonstrated leadership competencies as outlined in nationally recognized professional associations.

Special Training:Proficient computer skills – Microsoft Office Suite.

Mental, Behavioral and Emotional Abilities:Ability to build and maintain well-developed relationships with key members of the medical staff and physician leadership. Ability to effectively present medical information one-on-one and to small groups.

Essential Functions

  • Manages and develops Care Management - Acute Care staff.
  • Develops well-honed relationships with physicians, physician leaders, and other providers about the flow of patient care within CH.
  • With input from key stakeholders (Executives, Physicians, Clinical Staff), continually develops the work of Care Management team to ensure a patient-focused delivery system for care coordination and discharge planning.
  • Works with key stakeholders in the organization to determine metrics for success. Holds team responsible for outcomes.
  • Ensures that an appropriate staffing model is resourced, implanted and monitored to achieve outcomes.
  • Ensures compliance with Joint Commissions standards, NJDOH regulations, approved CH performance improvement plan and the HQSI activity for CH.
  • Develops recommended policies and implements approved policies an procedures.
  • Collaborates, assesses and makes recommendations to physicians and leadership to identify opportunities for improvement in patient care management and optimum resource utilization.
  • Provides information and analysis of performance measurement and resource consumption data as required.
  • Promotes and participates in care paths and other approved performance team activities as required.
  • Identifies educational goals of the staff. Develops and implements training plans. Interviews and hires employees, assigns work, appraises performance, and solves workflow and other problems.
