Job description

Scope of Work for Patient counsellors/ Patient Navigators

 

Commitment

Under the payor partnership, outlining our core purpose as one for patients and on closing the access gaps to provide the support on the patient counseling services to provide HNC and colorectal cancer patient counselling for psychological support, nutritional counselling, to educate, improve treatment adherence and reduce lost to follow up.

Objectives

1. Donation under Patient Benefit Develop operating model for patient counseling services and psychosocial support with a focus on Head & Neck and Colorectal cancers.

2. To educate HNC patients about early signs and symptoms and support them for timely diagnosis and treatment.

 

Methodology

In order to support the clinicians, the awareness is meant for the cancer patients to manage immediate counselling needs and medical management. Due to the high overload and scarcity of time there is no effort to understand the voice of the patient and bring the management of the conditions to a more community based rehabilitative process that provides support to the patients as well as the care givers.

The project envisages an outreach to the diagnosed cancer patients, survivors, and caregivers/families to provide counseling and psychosocial support to navigate the cancer journey and manage their condition better.

Patient counselor will be trained in psychosocial counselling support to patients and placed in the OPD of King George's Medical University, Lucknow . It will also use this methodology to address the access gaps with the larger patient audience to raise awareness on the challenges faced by patients and caregivers suffering from HNC cancers.

Annex A- :- Patient Counsellors (1 position)

Location: King George's Medical University, Lucknow

Position: Carer Counsellor- Oncology

Department: Oncology

Experience- 2-3+ years' work ex Post MSW or Master's in psychology.

Qualifications: MSW (Medical social worker) / Postgraduates (psychology) or equivalent

 

 Key Responsibilities

Support Physiological / Patients counselling on diagnosis; treatment navigation and supportive nutrition as per the advice/consultation of Oncologists, Clinicians; Direct Service to Patients and Families.

 Collaborate with the oncology department in the identification of cancer patients/families requiring psychosocial services.

Assess patients' and families' psychosocial needs and prepare counselling plan, in conjunction with the patient, caretaker and health care team, to address those needs and to enhance, promote and maintain optimal psychosocial functioning of the patient and family.

Prepare report on data generation with overall counselled, screening and patients follow up for treatment and adherence to treatment.

Support and help the patient cope with the emotional, psychological, social, and spiritual and functional aspects of cancer and its treatment.
