Job Description:

Contract Length: 13 weeks

Start Date: ASAP

Guaranteed Hours Per Week: 40

Primary Location: ThedaCare Heritage Manor

Shift Requirements: 8 8hr NOCs/Days /Variables

**all missed shifts must be made up within 2 weeks of the absence

Weekend: Every other

Holidays: As needed

Float Requirements: To all ThedaCare facilities

On Call/Call Back Requirements: None

Certifications: WI CNA AHA BLS Medication Administration Certification

Minimum Experience: 2 years LTC experience REQUIRED

Recent LTC facility experience REQUIRED

Charting Experience Required: EPIC Preferred

Required Skillset: Worked in LTC or Assisted Living

Community Based Retirement Facility

Patient ratios vary depending on CBRF or RCAC 1:61:15

Scrub Colors: Any scrubs (no hats or hoodies. Scrub jackets are okay)
