Job description
Qualification Requirements-
- Graduation in Science/paramedical background preferred – Nursing, MSW, nutritionist, BPTh, BOTh, Counsellor.
- For candidates from non-Science background past experience/ special educational qualification in counselling is mandatory
- Past experience in counselling preferred
- Should be fluent in English ,Hindi and regional languages Brief Profile PSP Counsellor (ALP) 1. Role of a counsellor involves meeting patients
- Meeting patient within 24 hours of receiving request to contact
- Sharing the essence of the Patient Support Program
- Sharing essential information about the disease, broad treatment guidelines and lifestyle modifications
- Carefully listening to the concerns and difficulties faced by the patient
- Emphasizing the importance of compliance to therapy suggested by the treating physician
- Timely follow-up with patients to assess patient progress towards therapeutic outcomes and also to ensure that the rapport and timely communication to the respective doctor.
- Analysis and documenting of reasons for drop-out.
- Taking doctor and patient consent, and proper maintenance of consent forms.
- Supporting queries from patients not enrolled on Saath 7, only if query is related to use of device
- Value Addition to Doctors
- Customizing the patient compliance program to the needs of individual doctors and ensuring timely and high-quality patient servicing, care and counseling.
- Sharing patient progress with doctors.
- Reporting
- Ensure collection of complete and reliable data
- Ensure 100% updation of database of all new and old patients
- Timely reporting of data
- Maintaining complete data confidentiality
- Pharmacovigilance and PTC:
- Report all Adverse Events [HIDDEN TEXT] within 1 working day.
- Report all PTC to [HIDDEN TEXT] within 1 working day.
- Work Flow
5.1 New Patient
- New patient contact should be first received by the Counsellor from the doctor/patient
- Once the patient is contacted by the counselor, it is the counselor's responsibility to give feedback on the same to the doctor as soon as possible.
- Visits to new patients at night should be avoided except in cases of absolute urgency.
- Sunday is the weekly off for the PSP team.
5.2 Follow-ups of Old patients
- All follow-up calls to be done on a regular basis as per the call plan. Patients to be called as per the last call date.
- Follow-up visit to be done as per program strategy
- Customized visits to be given to patients as per the need & recommendation of doctors
5.3 Doctor Co-visit
- To be decided in the beginning of the month and communicated to the PSP coordinator
- Record of number of patients continuing on therapy and their current therapeutic status to be shared with the doctor.(see appendix)
- Record of patients who have discontinued therapy should be shared with the doctor along with the reason for discontinuation.
- Each visit should end by asking the doctor for any special advice /instruction to be given to his/her patients. Reporting
