Job Title: Care Coordinator
Qualification: Graduation or equivalent degree.
Experience: Minimum 1 year in customer service with medical/clinical knowledge.
Working Days: 4 days shift and 3 days off (Rota Basis)
Work Timings :11.30am-10pm(As of now) and 10.30am-9pm(After March 2024)
Work Location: Chennai, India.
Expected Joining Date: Immediate Joining
Languages must be known: Fluent English (Knowing British Accent & and Hindi is a plus point)

Job Summary :
Book the right staff, update the system, communicate effectively with clients, family and ensure that the care staff delivers high-quality and personalized care to clients.
Key Responsibilities :
- Log in to the Connect Team App and read the updates.
- Record (write) all your shift activities in your daily logs (Regulation 17)
- Read and respond to emails professionally
- Liaise and correspond with CCG, GPs and another healthcare professionals as required or requested by the line manager.
- Answer all phone calls professionally and respond accordingly
- Be aware of staff profiles and competencies to ensure that the correct care staff is booked to deliver high quality and personalised care to clients.
- Read and understand the care plan and get to know all the clients. Have an understanding of clients healthcare conditions.
- Input new clients onto People Planner and Mobizio system and keep the system up to date with correct details.
- Monitor POC and team leader WhatsApp groups to update relevant changes such as medications.
- Always make sure sufficient travel time between calls
- Ensure no missed or late calls and ensure permanent rota sent to client/ NOK one week in advance.
- Liaise with Team leaders to ensure effective monitoring and delivery of PPE's and staffing.
- Organise the coordination of carers for hospital/ ad hoc training.
- Communicate with the clients, their family, care staff, and professionals effectively about any change in care needs and update the PP and Mobizio system.
- If any complaint/concern is reported, follow the management algorithm and if required inform your immediate line manager, and as per their advice, act upon it and try to resolve it.
- Promote care workers to do their Mobizio enter electronically, and wherever necessary, do the Manual entry and keep the system all calls recorded.
- Apply templating at PP and ensure all care staff/clients have a permanent rota.
- Produce a weekly report and workforce requirement and produce the report at the weekly HR meeting.
- Approve care staff annual leave ( except team leaders need manager approval), emergency cover request and cover with the suitable care staff, update the PP, and keep Clients and family informed.
- Send staff absence record and write appreciation letter through office team
- Produce the shift data analysis and send it to your line manager.
- Send daily night call monitoring report.
- Do a complete and thorough handover to the next shift coordinator.
