Position Description
The Clinical Client Navigation and Utilization Coordinator is a member of Kenora Chiefs Advisory’s Primary Care Team. This position is intended to work with patients from member First Nations to ensure culturally appropriate care is provided while they are patients in hospital and proper supports are in place for them post-discharge.
Position Title: Clinical Client Navigator
Department: Primary Care
Salary: Dependent upon qualifications and successful probationary period
Term: Full time – 1 FTE
Location: 240 Veterans Drive, 3rd Floor, Kenora, Ontario
Reports to: Reports to: Director of Nursing
Summary of Duties
Ø Engage with the patient and caregiver during the patient’s stay in the hospital
Ø Collaborate with client navigation staff and LWDH staff/management to enhance patient experience and health outcomes
Ø Attend daily bullet/multidisciplinary rounds, attend discharge planning meetings as required
Ø Develop a plan based on the patient’s goals, this plan will include culturally appropriate care decisions around treatment, discharge navigation and post-discharge care
Ø Provide Case Management for discharge of clients back to First Nation communities
Ø Follow-up with patient’s post-discharge to assess and verify compliance with the treatment plan, and to mitigate any potential issues
Ø Assist patients with navigation of the Non-Insured Health Benefits system for necessary post-discharge care
Ø Facilitate connecting the patient with post-discharge specialty supports, such as mental health or palliative care
Ø Establish relationships with Health Directors of First Nations communities
Ø Establish relationships with First Nation Community Programs
Ø Develop a framework for outcome evaluation, community consultation process, establishment of targets, review of progress, program adjustments and evaluation process
Ø Prepare summaries and reports after each session, from process content and evaluation as required by the Community Health Services Manager
Ø Participate in KCA Health Team activities as required
Ø Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate
Ø Additional duties as required
Qualifications
Ø Ability to work independently and in a team setting
Ø Background in nursing, social work, or other health discipline
Ø Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written and oral)
Ø A full understanding of Anishinaabe culture,
Ø the ability to speak Ojibway is an asset
Ø University degree or college diploma from a recognized post-secondary institution (health sector or social work) and/or experience working in health or social work sector
Position Requirements
Ø Valid driver’s license and vehicle
Ø Must be able to travel to KCA’s member communities when required
Ø Ability to effectively manage time and priorities
Ø Ability to work independently and within a team
