Reporting to the Primary Health Care Manager, Zone and/or Provincial and/or the Primary Health Care Director, the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in each respective portfolio.
The incumbent works closely with Primary Health Care providers and teams, stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives (e.g., chronic disease and wellness). The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies. The coordinator works with NSH Physician Leaders to support the coordination and activities of the Department of Family Practice.
The Primary Health Care Coordinator will work collaboratively with family physician's offices and other health care providers to support change management strategies that are inherent in the services plan for primary health care. They are responsible for supporting the work of Primary Health Care and the Department of Family Practice through practice facilitation and practice support program coordination. They will work closely with Family Practices to facilitate and assist with the measurement and evaluation of practice improvements and to share/promote the successes of these improvements internal and external to NSH.
