JOB DESCRIPTION

Rehabilitation Specialist
The Rehabilitation Specialist will provide Psychosocial Rehabilitation Services (PSR) to individuals as needed. this function will assist an individual in improving their functional abilities to the greatest degree possible in settings where they live, work, learn, and socialize. Rehabilitation counseling, skill building, and psychoeducational interventions provided through PSR are used to support attainment of person-centered recovery goals and valued life functions. Approaches are intended to develop expertise to overcome barriers caused by an individual's behavioral health disorder and promote independence and full community participation.
The Rehabilitation Specialist can provide services in both a group and individual modality. this function serves as part of a multi-disciplinary team providing group facilitation, individual services, relapse avoidance and crisis intervention.

Essential Responsibilities

»Facilitate psychiatric rehabilitation services in group and individual modalities, including wellness self-management services designed to develop or improve individual's personal coping expertise and basic living expertise training designed to improve individual's capability to execute the basic expertise critical to his/her recovery.

»Participate and complete the required training for PSR services for CORE (Community Oriented Recovery and Empowerment Services).

»Function as a member of the CORE services team in the program.

»Participate in other significant trainings.

»Participate in treatment team meetings, upon individual and practitioner request, for the purpose of facilitating CORE PSR services.

»Provide linkage and referral facilitation for services, resources, and tools on behalf of PSR recipients to support them in achieving their goals.

»Collaborate with individual and collaterals in developing, reviewing and modifying an Individualized Recovery Plan that identifies the medically necessary services that will support the individual's goals and identified path to recovery.

»Maintain electronic case records in a timely manner and complying with OMH and Medicaid regulations and agency policies and procedures.

»Coordinate the discharge planning process. Identify the resources and supports needed for transition to another program and make necessary referrals, linkages for treatment, rehabilitation and support services.

»Maintain a work environment in a clean, organized, and safe manner. Conduct everyday inspections of work areas to look for unsafe conditions

»Use all required safety devices and personal protective equipment.

»Ensure all accidents are reported to manager and Human Resources.

»Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.

»Maintain a clean and neat appearance consistent with agency standards and act as a function model

»Attend all required meetings and trainings

»Participate in training programs for professional growth as scheduled and approved

»Responsible for knowledge and adhering to Gateway Hudson Valley's Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies. Keep abreast of any changes or modifications to policies or regulations. Ensure that all staff are knowledgeable of applicable regulations, laws, and policies

»Perform other tasks as assigned

Qualifications

Required Qualifications required

»Be at least 18 years of age and have a high school diploma or equivalent. Other paraprofessionals with 1-3 years of significant work practice in a behavioral health setting or a bachelor's degree.

»

Preferred Qualifications required

» practice working or volunteering in the human services field.

» practice working with person with co-occurring disorders/illness.

»Must be able to sit or stand for 8 hours/day

»Must have a valid NYS Driver's License and maintain a driving record which meets the insurable criteria of the Agency's automobile insurance carrier
