LOCATION: Newmarket, York Region, ON – Crosslinks Homeless Initiative COMPENSATION: $54,853 - $65,578 Annually Prorated
CONTRACT DURATION: Ending October 6, 2025

 

LOFT Community Services is a unique and dynamic charitable organization that supports people living with complicated issues such as mental and physical health challenges, substance use challenges, poverty and homelessness. Serving approximately 19,633 annually and providing 1,861 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. To help fulfill its mission, LOFT is hiring a Case Manager .

Job Summary

The Case Manager provides a range of services that supports individuals living with serious mental illness, addictions and physical health issues in York Region and South Simcoe. The Case Manager’s goal is to empower clients, thereby increasing their independence, dignity and self-respect in a safe and secure environment. This position liaises with other community agencies and professional services.

The York Region/South Simcoe Community Case Management Program provides short-term support or intensive case management for transitional age youth, adults and older adults (also associated with LOFT’s Integrated Psycho-geriatric Outreach Program). We offer one-to-one support to assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.
