Clinician - CORE Services The Clinician is a member of the Rehabilitation Services Team!
In this job offer, you will provide services that are person-centered, recovery-oriented, mobile behavioral health supports.
The Clinician is responsible for the clinical services in the psychiatric rehabilitation program. Your services will help build expertise and self-efficacy that promote and facilitate community participation and independence.
Cliniciansprovide an array of psychosocial rehabilitation services designed to support individuals in restoring interpersonal, self-management and basic living expertise; prevent relapse; manage symptoms related to behavioral health conditions; monitor/address physical health conditions; strengthen wellness and self-care practices and achieving person-centered goals, in community settings.
You will collaborate with individual and collaterals in developing, reviewing and modifying an Individualized Service Plan that identifies the medically necessary services that will support the individual's goals and identified path to recovery.
Clinicians provide goal-oriented verbal counseling or therapy, including individual, group and family counseling or therapy for the purpose of covering the emotional, cognitive and behavioral symptoms of a mental health disorder or for engaging, motivating and stabilizing persons with co-occurring mental health and substance abuse disorders and the related effects on function functioning.

Qualifications
Job Qualifications for Clinician:
Education and practice : suitable education
Master's Degree in Social Work or related field. NYS license: LMSW or LMHC required.

Must have a Valid NYS Drivers' License Must have a favorable result from all state, federal, and agency mandated background check
