Under the direction of the Manager of Home Care or designate, provides an accessible client centered and integrated approach to the processing of requests for Homecare services; performs initial consultation, preliminary assessment and identification of needs for potential clients; takes and screens referrals from various sources: determines eligibility of clients, initial interventions and the response time needed. This position is a new position as such requires the successful candidate to identify improvements needed in existing process: and to use creativity and resourcefulness to help improve response time, and ensure appropriate and effective client care in the community. Integrating new and improved intake procedures into the present environment will be the key component of the position.

Recruitment, Relocation and Retention Incentives may be available up to $11,500 for a return for service commitment, along with a comprehensive benefit package.
