About the Opportunity


The Physiotherapist assesses, provides individualized exercise prescription, educates, and manages patients as part of a team, for patients referred to the Hearts and Health in Motion Program for primary and secondary cardiovascular disease prevention, pulmonary rehabilitation, and cardiovascular risk factor modification. The Physiotherapy assessments include obtaining a detailed medical history, exercise history, precautions to exercise, and risk stratification for subsequent events. Programs are developed using exercise, education, risk factor modification, and goal setting with motivational communication to assist the patient in regaining and/or maintaining an optimal level of function. The Physiotherapist provides organization for the exercise programs with monitoring and progression of patients. The Physiotherapist educates and counsels patients on exercise and activities outside of the program and works to meet patient-centered goals.


The Physiotherapist educates and communicates with other health disciplines, family members and caregivers, and the general public with regard to health promotion, physiotherapy treatment, and patient management. Reporting to the Program Lead, Health Services Manager, and Medical Director, the Physiotherapist is a member of a clinical team and collaborates with colleagues to determine team goals and objectives, and participates in research, the evaluation of program outcomes, team effectiveness, problem solving, and sharing responsibilities. The Physiotherapist supports the development of students through acting as a preceptor and providing ongoing teaching, supervision, and evaluation of physiotherapy students and supervision of volunteers.

 

About You


We would love to hear from you if you have the following:


-Graduation from a university with a degree in Physiotherapy or equivalent from a recognized school of Physiotherapy
-Current registration with the Nova Scotia College of Physiotherapy and eligibility for membership in the Canadian Physiotherapy Association
-Current BLS certification, including AED
-At least two years of recent clinical experience preferred
-Cardiac Rehab or Cardiac experience preferred
-Cardiology/Cardiovascular experience preferred
-Experience in providing virtual care preferred
-Strong communication and interpersonal skills with an approach of mindfulness
-Experience and/or training in motivational communication and behavior change techniques in both individual assessment and patient group teaching
-Experience and skill to work effectively as part of an interdisciplinary team. Experience working collaboratively with an interprofessional health care team to develop patient care plans is preferred
-Experience with developing and delivering patient group education, utilizing adult education and self-management support preferred
-Superior organizational, time management and problem solving skills preferred
-Training and experience in Motivational Interviewing and Motivational Enhancement practice preferred
-Research experience an asset.
-Ability to respond and cope with stressful or emergency situations
-Dynamic work environment includes the need to be flexible with programming changes to meet demands and team coverage
-Moderate to High amount of walking and physical activity on a daily basis in the exercise room and in travelling to and from community sites (includes lifting)
-Work area includes three program sites within NSHA with travel between sites (must have own vehicle)
-Able to work with patients in confined spaces between exercise equipment
-Able to lift and transport moderate to heavy equipment within and between sites
-Competencies in other languages an asset, French preferred

Compensation and Incentives


$33.31 - $42.06 Hourly
