General Summary
Reporting to the Senior Director of Wellness, the Nurse Practitioner (NP) is a primary care provider to the region assigned and works within their legislative scope of practice as described by the regulatory body. The Nurse Practitioner collaborates with the wellness team in the development, implementation, and evaluation of the residents' care plan; provides leadership and mentorship to wellness teams on assessment skills, evidence-based practice initiatives and is responsible for meeting the assigned region's policies, procedures, and protocols for medical services.
Principal Duties and Responsibilities
In collaboration with wellness team,
- Initiates treatment including healthcare management and therapeutic interventions and prescribes medications in accordance with statutory and regulatory standards, limits, conditions, and employer policies and procedures.
- Provides professional guidance to other health professionals and practices autonomously and interdependently withing the context of an interdisciplinary healthcare team, making referrals to specialist physicians and other as appropriate.
- Provides comprehensive primary care to residents as part of the interdisciplinary wellness team.
- Collaborates with residents and other health professionals to identify and assess trends and patterns that have implications for residents and families.
- Participates in peer and self-review to evaluate the outcome of services at the resident and residence levels.
- Collaborates and consults with physicians or other healthcare and social service providers as appropriate. Assists, supports, and guides Director of Wellness and wellness team members with external resources pertaining to resident care and challenging clinical situations.
- Makes referrals to specialized consultants and other health providers.
- Performs resident assessments including periodic reviews as per Company's standards.
- Liaises with discharge planning services for hospitalized residents to ensure a smooth transition back to the residence.
- Prescribes drugs withing the statutory and regulatory standards, limits, and conditions for Nurse Practitioners, and within applicable employer policies and procedures.
- Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
- Documents clinical data, assessment findings, diagnoses, care plans, therapeutic interventions, resident responses, and clinical rationale in a timely and accurate manner.
- Ability to work in a flexible environment to meet the needs of the residents, including weekend and holiday coverage, as required.
- Participates in interdisciplinary Team member education through case presentations, mentoring, role modeling, and facilitating the exchange of knowledge.
- Provides input to performance appraisals of wellness nurses.
- Engages in evidence-informed practice by applying relevant research, and theory in providing resident care services.
- Identifies and implements research-based innovations for improving resident care.
Job Specifications
Education
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM) in good standing.
- Successful completion of the Objective Structured Clinical Examination (OSCE) required.
- Recent, relevant clinical nursing experience
Experience
- Minimum (3) years’ experience as a Nurse Practitioner.
Knowledge & Abilities
- Ability to perform the full scope of NP duties and responsibilities.
- Ability to apply knowledge of pharmacology, pharmacokinetics, and pharmacodynamics to prescribing and evaluating treatment responses.
- Ability to communicate and collaborate with residents and families about health findings, diagnosis, treatment, self-care, and prognosis.
- Ability to critically assess and evaluate health research literature to determine best practices.
- Ability to introduce education and evidence-based research.
- Ability to identify and respond to legal or ethical issues that could arise during resident care.
- Leading and supervising team and providing feedback.
