Job Summary
Reporting to the Manager of Community Support Services, this position is responsible for the coordination of the Supports for Daily Living/Assisted Living program and case managing a range of services in collaboration with a contracted Personal Support Worker (PSW) service provider.
Duties & Responsibilities
- Work independently as a case manager in a leadership role with responsibility for day-to-day operations of the program and office location(s) while adhering to all required standards.
- Direct, coach and mentor contracted PSW staff and collaborate with the service provider Supervisor.
- Implement IPAC guidelines and ensure all PPE protocols in place to support client care
- Collaborate and maintain close working relationships with internal and external stakeholders (primary care physicians, Hospital, pharmacy, Home and Community Care Coordinators, local community support services, housing provider etc.)
- Coordinate and participate in home visits, care conference/hospital, staff meetings, administrative and relevant system–level meetings.
- Provide support to Martin House, Wellington Terrace, Bonnie Place I&II, and John R. Rhodes Residence.
- Complete InterRAI CHA assessments to determine service plan and support individual personal care needs through development and monitoring of client centered care plans.
- Support social and well-being activities for clients and building tenants.
- Monitor and mitigate employee health and safety risks onsite and offsite work locations (i.e. client homes).
- Verify and process contracted provider monthly invoice and manage monthly purchases.
- Prepare statistics, up-date forms, complete file notes and all related documentation.
- Implement, monitor and adhere to Corporate and program policies, procedures and protocols.
- Develop and participate in quality improvement initiatives.
- Performs other duties as assigned.
Skills & Qualifications
Essential
- A degree or diploma from a recognized educational institution in Gerontology and/or Health & Social Sciences.
- Three (3) to five (5) years related experience in case management for older adults.
- Strong care coordination skills and experience in conducting comprehensive assessments using the interRAI CHA (Community Health Assessment).
- Effective interpersonal and communication skills.
- Ability to work independently and in a team environment in a leadership role.
- Knowledge of the provincial Assisted Living for High Risk Seniors Policy, Multi-sector Service Agreements, Supports for Daily Living model of care and related Acts and Legislation.
Preferred
- Proficient in computer software applications and knowledge of Point Click Care (PCC).
- An equivalent combination of education and experience will be considered.

- Travel will be required, incumbent must provide their own transportation.
- Some evening and weekend work may be required from time to time.

- A current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including a Vulnerable Sector.
- A current two-step tuberculosis test (obtained within the past twelve (12) months.
