JOB DESCRIPTION

9:00 - 5:00 position M-F

POSITION RESPONSIBILITIES:
1. Work cooperatively with the Manager of Health/Nursing Services in assisting all individuals served to maintain optimal health.
2. Maintain client medication records, initiate and maintain contact with all individuals served, guardians, residences, and/or physicians to update medication records bi-annually and as necessary when changes occur.
3. Maintain CPR certification.
4. Provide first aid and nursing care to all individuals served and make appropriate referrals for treatment.
5. Administer medications as directed by physicians when necessary.
6. Provide and assist all individuals served with specific physical problems and coordinate that care with community agencies when appropriate.
7. Provide individual and group health maintenance instruction and counseling to all individuals served.
8. Maintain accurate health records on all individuals served in conformity with OPWDD regulations.
9. Maintain current clinical training and evaluations of staff on annual basis.
10. Maintain up-to-date knowledge base of research and developments for disabled population as it relates to health needs and medications, outside of training programs by researching and printing medication updates and potential side effects.
11. Liaison between staff members, medical care providers, and clients regarding interrupt, return to work and/or termination in terms of illness and proper physician approval to return.
12. Teach Blood Borne Pathogens, Infection Control, TB and Seizure Precautions courses as needed for staff and all individuals served.
13. Attend Residential Staff meetings and VTP meetings for medically involved individuals as the nursing representative.
14. Ensure appropriate overview of medical appointments, consults, medications, case notes and lab work in foothold.
15. Administer semi-annual medication review and annual nursing summaries.
16. Supervise and maintain AMAP annual pours for identified staff.
17. Participate in training programs for professional growth as scheduled and approved.
18. Perform other tasks as assigned.
19. Maintain a work environment in a clean, organized and safe manner.
20. Use all required safety devices and personal protective equipment.
21. Perform work tasks in a safe manner, and do not take unsafe ?short cuts?.
22. Conduct everyday inspections of immediate work area(s) to look for unsafe conditions.
23. Ensure all accidents are promptly reported to immediate manager and Human Resources.
24. Be familiar with the Safety and Health Accident avoidance Program and abide by all safety and health rules, work practices, and regulations.
25. Responsible for knowledge and adhering to Gateway Community Industries' Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies.

Qualifications

RN Licensed in NYS and three years of practice in indirect medical/surgical nursing; practice with a disabled population preferred. Valid NYS driver's license, must maintain a driving record which meets the insurable criteria of the Agency's automobile insurance carrier. Must have a favorable result from all state, federal and agency mandated background checks.
