Reporting to the Jr/Manager of Participant Services, the Program Coordinator (PC) is a full-time coordinator position responsible for providing support to MCL’s participant population and providing leadership to MCL’s direct support staff. The PC is also responsible for the daily planning, organization, and supervision of MCLs programs and services.
Regular PC working hours are 8:30am-4:30pm, Monday-Friday . However, the hours of work for the PC position are flexible and can be unpredictable to respond to participant and programming needs.
Position Overview
Participant Planning and Support
- Responsible for maintaining strict confidentially on all matters pertaining to the participants served.
- Responsible for working collaboratively with the participant and their care team(s) to ensure the Commitment to Care Plan is being upheld by all parties.
- Responsible for working collaboratively with MCLs direct support staff and participant treatment team(s).
- Responsible for providing direct participant support and documentation as required.
- Provides supervision to MCLs direct support staff to ensure that participants are attending all medical, mental health and therapeutic appointments and that treatment recommendations are implemented.
- Responsible for collaborating with MCLs direct support staff to complete Individual Program Plans (IPP) and quarterly reports, proper documentation for medical and mental health records and information from relevant others.
- Responsible for fostering positive working relationships with community partners.
- Ensures participant Special Needs Requests are identified and submitted to the Care Coordinator in a timely manner.
- Identifies and documents any concerns reported by participants and/or family members, informs the Jr/Manager of Participant Services and ensures that all action plans and necessary follow ups are completed and documented in a timely manner.
- Determines the level of intervention and/or support required in the event of a crisis or incident and take measures to ensure the appropriate level of support is provided to the participant. The PC will develop and implement emergency response protocols for the participants as is appropriate.
- Participates in the referral and intake process, completing Commitment to Care Plans (CCP) and discharge planning.
- Responsible for ensuring direct support hours are being fulfilled as approved by the DSP Care Coordinator.
- Provides leadership to MCLs direct support staff and management in the planning and implementation of extra staffing hours. Ensures the budgeted extra staffing hours are utilized effectively.
- Responsible for the review and revision of IPP’s, Quarterly Reports, Provincial Incident Reports (PIR) and submitting them to the Jr/Manager of Participant Services.
Qualifications
- Graduation from a recognized Human Services Program, post-secondary education in the human or social science field, or an equivalent combination of training and experience is required.
- Front line experience working with MCL is considered an asset.
- Previous experience in a community based small option home and/or supervised apartment program and a minimum of 2 years supervisory experience.
- Possesses all courses and certifications required to work in the sector.
- Previous experience in developing and managing the implementation of Individual Program Plans (IPP).
- Demonstrated ability to work from a strengths-based, solution and person-focused practice approach.
- Basic computer and word processing skills.
- Clean Criminal Records Check in relation to job requirements.
- Valid drivers license and proper insurance to transport participants.
