Job Description:
The purpose of the Social Worker position is to provide comprehensive psychosocial assessments, crisis intervention, resource resourcing and various interventions to individuals and their families. The Social Worker assists with discharge planning, advocates for patients and families and is a liaison with various community agencies.
The Social Worker participates as an integral member of the multidisciplinary health care team in the Maternal Child and Youth Department in the provision of patient care and must have strong interpersonal skills including a demonstrated ability to work independently and in a collaborative team setting.
The Social Worker must demonstrate ability to provide strong written and oral communication skills and accurate and timely documentation which meets both professional college and organizational standards as well as proficiency in multiple computer applications.
Sound knowledge of community resources and supports with a focus on maternal child and youth. Ability to manage time effectively and prioritize according to need and risk. Experience in facilitating group intervention.
Education/Requirements:
- Masters of Social Work degree from an accredited School of Social Work
- Registration and proof of good standing with Ontario College of Certified Social Workers/Social Services Workers mandatory
Experience:
- Minimum 5 years of progressive clinical experience in a health care setting
- Minimum 2 years working with Women, Children and Families
- Perinatal Loss Education and previous experience an Asset
- Minimum of three years clinical experience providing single session/brief clinical counselling to individuals, couples, and families
- Strong knowledge of various individual, couple and family interventions such as Cognitive Behavioural Therapy, Solution Focused Therapy and bereavement/grief counseling
Competencies:
- Sounds knowledge of the Child and Family Services Act III
- Understanding of the DSM IV
- Ability to work independently or in a team environment
