0.4 FTE (15 hours per week) Current schedule for this job share is Thursdays and Fridays. Current location: Downtown clinic. Responsible for creating an individualized plan of care, monitoring and evaluating individual’s program through the program, coordinating care, health promotion and ongoing support for individuals and their families.
Provide ongoing comprehensive primary health care, with a focus on mental health, in collaboration with an interdisciplinary health team to clients of all ages. Practice within the scope of practice according to the Saskatchewan Registered Nurses Association Registered Nurse (Nurse Practitioner) Practice Standards, including; diagnosis, counselling, screening, referral, education, treatment and follow-up in a clinical setting and in the community.
