Reporting to the Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.
Job Qualifications Required Qualifications
- Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing
- Basic Life Support (BLS)
- Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Experience -Minimum two (2) years of recent nursing experience in a community based setting is required. -Previous experience in teaching, facilitation skills and leading group activities is required. -Experience in case management is an asset.

- Demonstrated clinical competency including critical thinking skills and confidence to work autonomously.
- Demonstrated knowledge of chronic disease prevention and management.
- Demonstrates ability to accept and implement change
- Demonstrates empathy, trust and respect in the provision of care
- Demonstrates knowledge of population health promotion theory and practice and ability to transfer this knowledge into practice.
- Demonstrates sound evidence-based knowledge and judgment when making decisions about a client's nursing care
- Demonstrates the ability to manage conflict toward effective outcomes.
- Effective communicator with active listening and motivational interviewing skills.
- Knowledge and application of quality improvement principles, tools, and, outcomes.
- Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on chronic disease.
- Knowledge of the Lean Management System.
- Strong computer skills
- Systems thinking approach to health care delivery.
- Team leadership and facilitation skills including an understanding of the fundamentals of team dynamics.
