Ottawa Inner City Health is looking for a Mental Health Nurse Coordinator to join our team.
The Mental Health Nurse Coordinator is an advanced practice nurse who works collaboratively with Nurse Coordinators and partner agencies of the Ottawa Inner City Health Inc. in order to provide mental health assessments, develops and implements care plans and appropriate treatment options for patients living with mental health issues. The Mental Health Nurse Coordinator provides leadership and exemplifies the values of the Ottawa Inner City Health Inc. in his/her practice.
POSITION SUMMARY:
- The Mental Health Nurse Coordinator (MHNC) provides clinical care to clients who receive their health care at the Ottawa Inner City Health Inc. programs:
- The MHNC)identifies homeless persons who may benefit from mental health services and engages them in a therapeutic relationship.
- The MHNC completes a comprehensive mental health assessment using a holistic approach which may include administration of the MoCA and PANSS tools.
- The MHNC provides information to the clients about the Ottawa Inner City Health Inc so they are fully informed about services available and facilitates the process of obtaining informed consent so the client can participate in his or her own health care.
- The MHNC formulates a plan of care in partnership with the client and the other health professionals involved in the care of the client.
- The MHNC consults with the attending psychiatrist when appropriate and follows his/her plan of care when one is established.
- The MHNC monitors compliance with the plan of care and insures that the clients’ wishes are understood and respected.
- The MHNC acts as an advocate on behalf of the clients.
- The MHNC provides learning opportunities for health care students, front line staff and Nurse Coordinators.
- The MHNC participates in research and evaluation as appropriate and collects data for funding purposes.
- The MHNC provides day to day supervision of the client care workers and students assuring adherence to the policies, procedures and practices of the Ottawa Inner City Health Inc. and partner agencies.
- Maintain accurate patient records.
- Participates in the afterhours on-call rotation, providing support to front-line, nursing and personal support staff members in order to provide high quality health care at all hours.

QUALIFICATIONS:
- The MHNC must meet the standards of practice of the College of Nurses of Ontario.
- Possess a current Registered Nurse, Certificate of Competence.
- Has attained a minimum of a Bachelor’s of Science in Nursing, Masters preferred.
- A minimum of 2-5 years’ experience working directly with persons who are homeless and living with persistent mental health and substance use issues.
- Demonstrated competence in clinical nursing practice related to the care of adult homeless persons.
- Demonstrated ability to work with other care professionals in care delivery.
- Demonstrated ability to teach, consult and negotiate.
- Guided by the service delivery philosophy and the mission of the Ottawa Inner City Health Inc.
- The MHNC must have strong organizational and leadership skills.
- Familiarity with Electronic Charting Systems, proficiency with Microsoft Office software.
- Have a stable and positive attitude towards his/ her work.
- Be a self-learner and self-directed.
- Adopt an independent practice model of care delivery while working cohesively with the nursing team.
- Understand how to apply harm reduction strategies in the clinical setting.
- Be proficient in trauma informed care.
- Current Standard First Aid certificate.
- Must provide a Vulnerable Sector Criminal Records Check (A criminal record is not an automatic disqualification for the role).
