The Systems Navigator provides intake services for new clients, and provides a comprehensive assessment of their strengths, capacities and needs, based on the social determinants of health. They will assist clients in navigating the broader health care system while also establishing and maintaining a strong relationship with community partners. This position offers a unique opportunity to make a difference in the lives of newcomers and refugees, and those significantly marginalized by poverty, addiction, mental health, and housing issues. This role will work with our newcomer clinic, supporting newcomers to Canada along the continuum of care within a health promotion framework and liaising with interdisciplinary team members and external care providers to facilitate the best plan of care for clients.
