Indigenous Transition Facilitator
Summary
This resource will seek to integrate provincial, federal and other community health and social support services across the care continuum. The ITF will help support the client and family throughout their health care journey transitions by facilitating smooth transitions in care, contributing to better health outcomes for Wikwemikong clients. Transitions of care: refers to the various points where a patient moves to, or returns from, a particular physical location or makes contact with a health care professional for the purposes of receiving health care. This includes transitions between home, hospital, residential care settings and consultations with different health care providers in outpatient facilities.
Responsibilities
- Be in contact with each community member accessing medical services in hospital to see if health/social care needs are required. Providing support and advocacy to community members via a team approach by acting as a liaison between client/family and health care team.
- Attend medical rounds at local hospitals including Health Sciences North and Manitoulin Health Centre.
- Case management/care coordination services to manage transitions of care.
- Liaising with service providers and suppliers to arrange and authorize necessary services, equipment and devices, medical supplies, and transportation.
- Ensure the early engagement of client/family in development of the care plan.
- Determine any education or training they may require to support the client in their living arrangement of choice upon discharge from a regional facility.
- Relationship building across multidisciplinary teams and community partners formalizing process to strengthen efficient client transitions across the care sector; acute care to community and long-term care.
- Utilizing effective communication strategies, shared care plans, family meetings, and electronic health record entry.
- Receiving and processing all referrals and inquiries and providing clear and accurate information on available services to potential service users, related support networks, service providers, and community members, outlining what they can and cannot expect from certain services or care plans.
- Participate in the Indigenous Transition Facilitator Community of Practice teleconferences to stay current on best practices and establishing networking connections with other First Nations Indigenous Transitions Facilitators.
Qualifications
- Registered Nurse/Registered Practical Nurse in good standing with the College of Nurses of Ontario OR Social Worker in good standing with the OCSWSSW, or other allied health professional with relevant skill set.
- Minimum of 2-5 years nursing or social work experience.
- Experience in case management is preferred.
- Indigenous cultural competency training (organizational curriculum of choice).
- Indigenous lived experience.
- Ability to speak an Indigenous language is an asset.
- Knowledge of system navigation, community resources, legislation, research and funding arrangements specific to Indigenous programs and services.
- Ability to assist and support in processes of engaging and building mutually supportive relationships with local Indigenous communities/organizations in a community-centered manner.
- Experience in an Indigenous health and or social service organization, hospital or community-based case management, health promotion, public health or social work is essential.
- Demonstrated knowledge of health care system as it relates to Indigenous communities, or willingness to learn and engage with individual communities as required.
- Demonstrated problem solving and critical thinking.
- Demonstrated time management and ability to work autonomously.
- Excellence in communication, partnership building and inter professional care commitment.
- Proficiency in the use of computers and various software applications including electronic health records applications
- Demonstrated commitment to continuous learning and professional development
