Unit Summary

The Integrated Stroke Model of care provides assessment and treatment to stroke patients along the continuum. The stroke system begins in acute care on the 24 bed unit on 7 South at the Hamilton General hospital site. Stroke recovery continues on B2North, a 28 bed unit in the Regional Rehabilitation Centre co-located at the HGH site. The team also works closely with community partners to support ongoing integration into the community. Staff will have experience with skills in the treatment of stroke patients in both the acute and rehabilitation phases of the care continuum to support quick and seamless transitions between units. This unique model provides improved access to best practice and rehabilitation care for all individuals who have suffered a stroke.

As a member of the interprofessional rehabilitation team, the Registered Nurse practices in accordance with relevant legislation and standards that meets the legal and ethical requirements of the profession. The primary responsibilities include: planning, managing, evaluating and coordinating comprehensive professional nursing care for patients and families within the context of an interprofessional practice; using theoretical and research knowledge and the input of team members to plan and implement care including health promotion for stroke rehabilitation patients and families; educating patients and families regarding stroke rehabilitation; participating in interprofessional team care planning discussions, rounds and family conferences; providing leadership, supervision and mentorship to students and new staff on the unit related to stroke rehabilitation; participating in professional development programs of self and others and rehabilitation initiatives, compliance with and commitment to patient and staff safety responsibilities and corresponding organizational plans, policies and procedures in order to ensure a safe environment for patients and staff.

1. Baccalaureate degree in nursing strongly preferred.
2. Current Certificate of Registration from the College of Nurses of Ontario as a Registered Nurse in good standing.
3. Recent 2 to 3 years’ experience in stroke rehabilitation or neurological nursing required.
4. Successful completion of Basic Cardiac Life Support (C level) Heart Saver Plus Heart and Stroke Basic Life Support or equivalent Health Care Provider course.
5. Demonstrates knowledge of Canadian Stroke Best Practices and how they are integrated into daily rehabilitation nursing practice.
6. Demonstrated knowledge of in-depth health and physical assessment.
7. Excellent conceptual and theoretical understanding of nursing concepts and ability to articulate the concepts as they relate to stroke rehabilitation.
8. Excellent clinical decision-making skills that reflect patient centeredness, problem solving and critical thinking.
9. Excellent interpersonal, leadership and communication skills (verbal and written).
10. Demonstrates ability to apply theoretical concepts and research findings in practice.
11. Demonstrates ability to teach students within the context of an academic health care setting.
12. Demonstrates ability to work independently, and as part of an inter-professional team.
13. Demonstrated collaborative approach to daily interactions that support respectful and effective working relationships with clients, families, the rehabilitation team and the community.
14. Excellent knowledge of the standards of rehabilitation nursing practice and applicable health care legislation.
15. Demonstrates willingness to gain knowledge and acquire skills to develop a higher level of professional expertise in stroke rehabilitation including creating a stroke specific learning plan based on stroke core competencies for nursing.
16. Demonstrates a willingness to learn and ability to identify and meet own learning needs for continuous professional development.
17. Ability to articulate and provide substantiated rationale for clinical management of patient care.
18. Computer proficiency and electronic documentation required.
