POSITION PURPOSE
Reporting to the Director, Housing Strategy & Program Delivery, the Social Worker is responsible for the assessment of and intervention with, clients and their families, assisting them to obtain the most appropriate services and enabling clients to maintain their independence and quality of life; involvement in a diversity of issues related to programs delivered by the Community & Health Services Department; developing and maintaining positive and cooperative partnerships in the Department with local community service providers and clients.
MAJOR RESPONSIBILITIES
- Facilitates/mediates the resolution of difficult, complex problems the client is facing which are resulting in barriers to service access, maintenance and/or self sufficiency (e.g. physical, mental health financial, housing/tenancy, abuse, bereavement, complex family dynamics, community problems, etc.) by working with individuals and families; investigating and determining causes, outlining implications regarding their future status and developing options/recommendations to de-escalate crisis situations and resolve issues.
- Provides instrumental and supportive counselling and case management to assist seniors/families to function safely and as independently as possible in the community.
- Provides intervention in urgent client situations; completes assessments and recommends solutions, provides information, referrals, advocacy and system navigation of community programs and services as required.
- Identifies unmet service needs, explores alternatives for service provision, and participates in the development of new programs, as applicable.
- Maintains records, documents, action plans and ensures confidentiality of client information.
- Assists the manager in ongoing service planning and program evaluation.
- Develops partnerships and maintains ongoing liaison with Regional delivered services, community and social agencies by facilitating and/or attending multi-agency (e.g. health, social, community, recreation, and local law enforcement) meetings; supports and encourages Departmental staff to develop and maintain awareness of community issues and programs.
- Initiates/chairs or participates in case conferences; ensures that documentation and plan of action is completed; refers clients to other services where appropriate and maintains follow-up records.
- Performs other duties as assigned, in accordance with Branch and Department objectives.
QUALIFICATIONS
- Successful completion of a University Degree in Social Work from an accredited university.
- Must be a Registered Social Worker with the Ontario College of Social Workers and Social Service Workers.
- Minimum two (2) years experience in the field of social work in counselling/case management and demonstrated skills and experience in working with older adults and families.
- Satisfactory Vulnerable Sector Check.
- Valid Ontario Class “G” driver’s licence and reliable vehicle for use on corporate business.
- Computer literacy utilizing MS Office software applications, as well as proficiency in word processing, spreadsheet, scheduling and presentation software.
- Knowledge and demonstrated ability in corporate core competencies including communication, innovation, teamwork and collaboration, and personal ownership.
